In purchasing an on-line ticket, you confirm that you are 18 years or older and have read, understand and consent to the terms of this Y.T.I. Participation waiver agreement. The ticket purchaser voluntarily agrees to participate in the Annual Family Bowl Fundraiser sponsored by Y.T.I. Youth Tutoring Initiative on September 21, 2019. The undersigned recognizes that Y.T.I. Youth Tutoring Initiative has not undertaken any duty or responsibility for his or her safety and the undersigned agrees to assume the full responsibility for all risk of bodily injury, death, disability, and property damage as a result of participating in the Annual Family Bowl Fundraiser.
Warranty of Physical Fitness: I warrant that I am physically fit and in a condition that will allow me to participate fully in the activity. I maintain medical insurance that covers me for accidents and illnesses while I am participating in this activity. I understand the Y.T.I. Youth Tutoring has not made, nor will make, any investigation into my physical fitness or ability to participate in the activity, and Y.T.I. Youth Tutoring Initiative is relying on my warranty of my physical condition. I assume full responsibility for payment of medical expenses not covered by my insurance incurred as a result of my participation in the activity.
Y.T.I. Youth Tutoring Initiative is released from any and all liabilities, claims, demands, actions, causes of actions, costs and expenses of any nature whatsoever arising out of any loss, damage, or injury, including death, that may be sustained by me or to any property belonging to me, arising from the bowling activity or while upon the premises where the bowling activity is being conducted. This agreement also binds the members of my family and spouse (if any), my estate, heirs, administrators, assigns, and personal representatives.
I agree to indemnify and hold harmless the Y.T.I. Youth Tutoring Initiative from and against any loss, liability, damage or costs, including court costs and attorneys’ fees, that Y.T.I. may incur arising from my involvement in the Activity.
In purchasing a ticket, I, the purchaser, recognizes that participating in the above activities, may expose me to several inherent risks, including but not limited to those listed below:
• Injury to Feet & Hands (E.G. Smashed, Broken Bones, Bruised)
• Pulled Muscles
• Twisted Ankles
• Scratches and Bruises
• Joint Injuries
• Back Injuries
• Heart Attacks
By purchasing an on-line ticket to the Y.T.I. Annual Bowling Fundraising event, I hereby state that I am 18 years or older, or a legal guardian of the participating minor and understand the risks involved in participating in the bowling activity and willingly and voluntarily accept these risks. By purchasing a ticket, I hereby surrender any right to seek reimbursement from Y.T.I. Youth Tutoring Initiative and its directors, officers, employees, volunteers and other agents for injury sustained and liability incurred during my participation in the activity described above. When purchasing tickets to the event I assume responsibility for my own safety and I understand and accept the risk involved with my participation.
